I recall that it was William and Ari that designed the world of Hollow Knight and wrote the dialogue together, so it would seem that the two of them specifically have liberty over what does or does not end up in the game. I do believe that in one of their interviews, Team Cherry commented on their team size and stated that they wish to keep it small so that they can maintain a greater degree of creative control that isn't split between too many different people.
